Identity & Statement of Educational Purpose

Students may be selected to verify their identity and sign a statement of educational purpose. If chosen to complete this process, financial aid will not be determined at Empire State University until this requirement has been satisfied. If requested, please download the appropriate form below. Please note copies, emails or faxes will not be accepted.  We must obtain the original completed form along with a copy of the unexpired government-issued ID.  

Form A is for students who reside in New York state or are seeking admission to the following Empire State University locations: Binghamton, Brooklyn, Buffalo, Corning, Fort Drum, Harlem, Manhattan, Newburgh, Rochester, Saratoga Springs, Selden, Staten Island.
